MDCP Application Kit 2009 The most important part of the application kit is the
notice of
Federal Funding Opportunity (FFO) for 2009. A notice announcing the
FFO was published in the Federal
Register. The FFO describes application requirements and format, specifically:
(1) Executive Summary, (2) Background Research, (3) Project Description,
(4) Credentials, (5) Finance and Budget, (6) Forms, and (7) Appendices.
The application kit below covers only instructions, samples,
and forms regarding application sections (5) and (6), Finance and Budget,
and Forms. There are basically five steps required to submit your MDCP application online. 1. Register on grants.gov. 2. Use the application kit below as a guide in developing your application. 3. Convert or scan your application to pdf file format with sequentially-numbered pages. 4. Log onto grants.gov and transcribe information from your application such as from form 424 and 424A, into the online form that comes up. 5. Attach the pdf to the online forms that grants.gov requires and submit. Grants.gov should send you a confirmation email right away.
[1] One computer file contains several linked spreadsheets. Each spreadsheet is a different document. [2] Develop the contents of this form using the mock application as a template. [3] Create your own budget worksheet that links to the Budget for Project Award Period. You may modify the worksheets used in the mock application or start from scratch. [4] Complete this form only if you plan to subcontract at least $25,000 of your project. Do not submit. Keep it on file. Any subcontractor for $25,000 or more will need to make the certification required by this form.[5] Complete if, as explained in CD-511, 3.(2), the MDCP project includes lobbying the federal government.
